FormDr uses the Patient.Create, Patient.Read, and Patient.Search to search for patients, create them in the system, and update specific fields in the patient chart. The integration is unidirectional, so new patient submission data will be sent into Epic from FormDr.

No, FormDr does not charge to setup the integration.
