To Bulk Export Submissions:

1. Log in to FormDr
2. Select ‘Integration‘
3. Select ‘Bulk CSV Export of Received Forms’

Bulk CSV Export

Easily export patient submissions from a specified date range and select from which forms. The CSV will be formatted so the column title correlates to your form’s questions. Each row beneath the column title is a patient’s submission. Select the date range (from date/to date) followed by the forms you would like to export from under ‘Filter by forms.’ If no date range is selected, an export of all submissions from all forms will be generated.

*NOTE: You can only export 30 days at a time. 

Export with Custom CSV Column Names

Need to name a column differently than what is on your form? Simply go to ‘My Forms’ > ‘Edit Forms’ to pull up the builder. When editing forms in the builder, select the field that needs the new CSV column name. Select the ‘edit’ button on this field, in the ‘Inputs’ area you will see a ‘CSV field name’ – type the custom column title in this area and publish your forms.
