Where do I request support?

To Request Support:

1. Log in to FormDr
2. Select ‘My Account‘
3. Click on ‘Support’

Requesting Support 

When reaching out to support use the drop down to select a topic. Once you have selected a topic use the comment box under 'What's your question, comment or issue?'. After you have submitted a ticket our team will respond to any questions you may have.
