Creating A Blank PDF To Give To Patients

To Create A Blank PDF:

1. Log in to FormDr
2. Navigate to the ‘Packets + Forms‘ page
3. Select the ‘…’ > ‘Settings’
4. At the very bottom is a button titled ‘Create PDF’

Create a Blank PDF

The blank PDF may be generated from any of your online form packets. The PDF will include all of the fields that are on your forms as well as all of the options included in each field. The blank PDF will give your patients the ability to fill out a paper copy of your forms.
