Exporting A Submission As A CSV File

To Export A CSV Of A Submission:

1. Log in to FormDr
2. Select ‘Send + Receive‘
3. Scroll down to the patient submission you would like to export
4. Select the ‘CSV’ button

Exporting as a CSV

You can export a single patient submission as a CSV file. The CSV will include all of the questions in your form, and the answers completed by the patient.

Most 3rd party services allow for a CSV upload if you would like to move this patient’s data to a different service. You can also bulk export multiple patient’s submissions in a single CSV file by navigating to the ‘Integration‘ tab.
