To Print A Patient Submission:

1. Log in to FormDr
2. Select ‘Send + Receive‘
3. Scroll down to the patient submission you would like to export a single form from
4. Select the ‘Print’ button, this will load a new Print page
5. In the top right corner of the ‘Printable Forms’
6. Hide the forms you do not want to be exported by clicking on them
7. Select either ‘Print’ or ‘PDF’ to export only the shown forms

Exporting a Single Form

You can hide as many forms as you need. When a form is hidden, you will see the eye ball icon with a slash through it. Forms that are shown will have an eyeball icon. As you hide forms, they will not be shown in the preview beneath. Once you have the forms hidden that are not needed, simply click the ‘Print’ button or the ‘PDF’ button to generate an export.
