To Send A Text Message Invitation:

1. Log in to FormDr
2. Navigate to ‘Send + Receive‘
3. At the top, enter the information into ‘Send Forms’

Send an Email Invitation

Quickly send a email invitation to your patients. Simply navigate to ‘Send Forms,’ type the patient’s name and email. Select the forms you would like to send and click the ‘Send Forms’ button. Your patient will immediately receive a link to your forms to their email address. You can track your patient’s progress as a patient opens your forms, saves their progress, and completes your forms.
