To Add Conditional Logic:

1. Log in to FormDr
2. Navigate to the ‘Packets + Forms‘ page
3. Select the ‘…’ > ‘Edit Forms’
4. Select ‘Next to Build’

Creating Conditional Logic Fields

Conditional fields will show or hide additional fields depending on what is selected. Most form elements have a conditional option with a dotted box area that says ‘Drag & drop form elements here.’

Simply drag and drop the fields you want included in this conditional area, click done, and be sure to publish your form.

The following form elements can be made conditional:

• Single-Select Options
• Multi-Select Options
• Dropdown
• Single Checkbox
