- Log in to FormDr
- Navigate to the ‘My Forms‘ page
- Select the ‘…’ > ‘Edit Forms’
- Select ‘Next to Build’
Adding a Signature to Your Form
To add a signature, simply select the ‘Signature’ widget under ‘Sign & Date.’ When adding a signature, you can adjust the alignment, and make the signature required. If you make the signature required, patients will not be allowed to submit your form unless they have signed this signature field.
The signature field enables your patients to sign your forms from any device, anywhere. Patients can sign from their phone, tablet, or computer before submitting their completed forms back to you.
