To Use Groups And Presets:

1. Log in to FormDr
2. Navigate to the ‘Packets + Forms‘ page
3. Select the ‘…’ > ‘Edit Forms’
4. Select ‘Next to Build’

Using Groups

Groups will give you more control over managing the elements on your forms. Select ‘Group’ under ‘Formatting’ in the builder. The group will load a blank area onto your form that states ‘Drag & drop form elements here.’

Once adding your first group, you can add form elements directly into the group. Groups allow you to do the following:

• Required – making a group required will ensure that a patient completes at least one element out of the group
• Duplicate – duplicate multiple fields at once by loading them into a group first, then selecting duplicate
• Title – the title of a group will display only when a patient misses a field in a required group. Upon attempted submission, the patient will be notified that a field was missed in the ‘Title’ of this group.
• Move – move multiple form elements to a different form in your packet
• Presets – load predefined fields from a list
