To Add Form Elements:

1. Log in to FormDr
2. Navigate to the ‘Packets + Forms‘ page
3. Select the ‘…’ > ‘Edit Forms’
4. Select ‘Next to Build’

Form Elements

The form elements are the primary tools used to collect patient information. Using the form elements you should have everything you need to collect completed forms online from your patients. Below is a list of the form elements:

• Patient Name – this important field needs to be placed at least once in your packet, your account will reference the patient’s name when collecting and storing patient information
• Text Field – a simple field for the patient to input a line of text
• Large Text Field – a multi line text field for the patient to input several lines of text
• Single-Select Options – give the patient the ability to select from one of multiple options
• Multi-Select Options – give the patient the ability to select several of multiple options
• Dropdown – a list of options displayed in a field that drops down for the patient to select
• Single Checkbox – a checkbox that may include a title
