The Rating Scale widget allows you to set up a list of questions with predefined answer choices.  

Use this widget to

• Survey clients with a predefined range of choices. 

1. Widget Options
   • Required: Turns on requirements. The form cannot be submitted unless this information is filled out. 
   • Hide on Print: This will hide the widget when selecting print or downloading the PDF. 
   • Hide from users: Allows you to hide this widget on the live version of the form. 
2. Title: Used to name the widget.
3. Name: This area is space sensitive and should not include space between words. 
4. Questions: Use this area to type in the questions you want to ask your patients. 
5. Question Name: This area is space sensitive and should not include space between words. 
6. Add Question: Allows you to add additional questions to the Rating Scale.
7. Ratings:
   1. Satisfied/ Unsatisfied: Default, predefined answer choices.
   2. Agree/ Disagree: predefined answer choices.
   3. Good/Poor: predefined answer choices.
   4. 1 to 5:  predefined answer choices.
   5. Custom: Allows you to create custom answer choices. 
8. Move: Use this tool to move the widget to another form within the same packet.
9. Delete Field: Will delete the widget from the form. 
10. Duplicate: Will create an identical copy of the current widget.
11. Collapse Widget: The "Pencil" button or "Done" button will collapse/close the widget.
