To Merge Patient Records:
- Log in to FormDr
- Select ‘Records‘
- Hit the ‘Select’ button, this will load a checkbox next to each patient record
- Select the patient records you would like to merge
- Select the ‘Merge’ button
Merging Records
In some cases you may have duplicate records of the same patient. When merging records together, you may be asked to select the values you want to keep if there is conflicting information. All fields in the demographic area will be compared for conflicts.
Once merged, you all submissions, notes, images/files, and history will be combined together into a single record.
Reversing a Merge
If you merged two records by mistake, you may have access to unmerge by going to the newly merged records history. There will be an option at the top of the history to select the ‘Unmerge’ button. Selecting ‘Unmerge’ will restore the two merged records. If you have began making changes to the newly merged record, you will no longer have access to unmerge.
