To Embed Your Forms On Your Website:
- Log in to FormDr
- Navigate to the ‘Packets + Forms‘ page
- Select the ‘…’ > ‘Settings’
- Scroll down to the Link/Embed Forms on Website
Embedding Forms on Your Website
There are two options to embed you forms directly on to your website. The first is with a simple link, you can adjust the text that will be hyperlinked, but you will need to leave the URL contained in the ‘href.’ The link will direct patients to your forms URL where they can fill out the forms online. This link will open in a new window and is separated from your site.
The second option is to embed the forms with an ‘iframe.’ The iframe allows you to place your forms directly onto your site instead of being directed to an external page. You can adjust the height and width of the iframe so it matches the formatting of your website. All of your forms will scale down to a mobile size as well.
