To Cancel Your Business Plan or Legacy Plan (Starter, Pro, Elite) Account:

1. Log in to FormDr
2. Select ‘My Account‘
3. Scroll down to ‘Delete Account’
4. Select the reason for closing the account
5. Leave a message (Notes for FormDr)
6. Click 'Delete Account'

Delete Account

You can delete your account at any time. Simply click the ‘Delete Account’ button, and you will be prompted for a reason why you are deleting and provided an area to leave brief description. Please note that only the account administrator can delete the account.

If on a Platform, Teams or Enterprise Account Plan

If you are on a Platform, Teams or Enterprise plan, please reach out to your account representative to assist you with cancellation.
