Form revisions are only included with the Enterprise plan.
WHEN SUBMITTING FORM REVISIONS:
If you are on the Enterprise account plan:
- Submit form revisions in writing to account representative OR
- Submit form revisions in writing to Support@FormDr.zendesk.com with the following information:
- Packet Name(s)
- Description/Notes: Include any revision notes or attached files (PDF or Word document preferred)
If you are on a Business or one of our legacy account plans (Pro or Elite):
- Revisions are only included with the Enterprise plan however, if you would like for our team to make updates to your forms you can upgrade to the Enterprise plan by going to My Account > Subscription.

Edit Your Form: To update existing forms yourself, please go to Packets + Forms > Edit Form (three dots on the right side of the packet) > Next to Build. Once in the builder, you will be able to add new widgets from the left sidebar by clicking them or dragging them from the left sidebar onto the page. Revision Submission Guidelines
Please see the following guidelines for our revision process. This is to ensure we are completing work in a timely manner and helping ensure accuracy:
Submitting Revisions: Enterprise accounts can submit revisions either to their account representative or they can send them to support@formdr.zendesk.com.
All form revisions must be submitted in writing. This is a requirement to avoid any miscommunication regarding the form to be edited, what edits are to be performed, and the exact wording that is to appear in the form.
Preferred File Types: Preferred file types will be MS Word or Adobe PDF files to allow the build team to copy and paste to ensure accuracy, as well as to speed up the revision process.
Form Revision Expectations: All forms will be created from the contents of the file submitted and will be built to be as close as possible to the original.
Translation Conversions (ex. Translating an English form to Spanish): While we do not offer translations services, if you have a translated form we can help convert that file however, this would be uploaded and charged as a new build ($10/page).
