- Name: Enter the name of the patient/client you wish to send the form to .
- Select Forms: Select the packet you want to send. (You can only send one packet at a time)
- Phone Number: To text the forms out, enter your patient/client's number here. You can send an email or text at the same time or just send an email/text. You do not have to enter both.
- Location: Select the location you want to send the packet from. This submission will go directly into that child account.
- Email Address: Quickly send a email invitation to your patient/clients. You can send an email or text at the same time or just send an email/text. You do not have to enter both.
- Appointment date and time: Select the day and time of the patient/client's appointment.
The SSO Admin has the ability to see all submissions within their network. You can use the following filters to sort submissions.
- Filter by Account: This will filter all submissions by the child account.
- Filter Received Forms:
- Packets – At the bottom of the filters are all of the packets that are assigned to your account. You can select a packet to only show submissions from that packet.
- Completed – Displays forms that have been completed by patients.
- Incomplete – Displays sent forms that have not been completed by patients.
- Opened – Displays sent forms that have been opened by the patient, but not completed.
- Received but not opened – Displays sent forms that were received by the patient, but the patient has not opened the link.
- Sent but not received – Displays sent forms that were not received by the patient. Not received forms occur when the email address or phone number that was used to send to the patient was incorrect or not available to receive text and email.
- Search Received Forms: Search by patient/client name.
- Date Range: Filter submissions from a date range.
- Received Submissions: This area will contain all submissions that have NOT been archived.
