To Format Forms:

1. Log in to FormDr
2. Navigate to the ‘Packets + Forms‘ page
3. Select the ‘…’ > ‘Edit Forms’
4. Select ‘Next to Build’

Formatting Your Forms

There are a number of elements that can be used to format your forms. Below is a list of what is included in ‘Formatting.’

• Title – titles enable you to break up your form and add titles to different sections
• Paragraph – place a large amount of text in a paragraph. The paragraph has many settings similar to what you would find in a traditional text editor.
• Columns – add multiple form elements into columns, stack form elements next to each other
• Page Break – add a page break so your form begins printing on a new page
• Group – load multiple fields into a group, move the group so all elements move at once, or require the group so patients may not submit with out selecting something in the group
• Popup Paragraph – similar to the paragraph, but text is hidden in a pop up that displays once the button is selected
